Casual Specialist Aged Care Registered Nurse Phone Support

This Package includes:

  1. Affordable 24 hour access to your dedicated Pathways Facilitator, a Registered Nurse with over 15 years experience in aged care.
  2. Charged at an hourly rate in 15 minute intervals.

Sometimes a brief chat is all a family needs to point them in the right direction, where they have chosen to manage the transition process themselves.

Carer Training

Upskill yourself to care for your loved ones during their time of need

This Package includes:

  1. Your own dedicated Pathways Facilitator, a Registered Nurse with over 15 years experience in aged care.
  2. Pathways Family Meeting at your home to discuss clinical care needs & personal care preferences, and care training required.
  3. One-on-one carer training with your Pathways Facilitator, such as safe Manual Handling practices, showering techniques, managing medication, end-of-life care.
