Heather Hill Pathways was founded in 2011 by Heather Hill, a Registered Nurse who has worked extensively in the community and aged care sectors. Through her nursing experience, Heather saw the emotional, financial and legal distress that many families experienced in making decisions about aged care.
It was her belief, that if families had easier access to clear information and relevant support, much of this distress could be prevented.
Motivated by the desire to assist families, Heather collaborated with a select group of aged care experts to provide affordable access to high quality, integrated aged care advice and planning.

Heather is a Registered Nurse with more than 15 years experience in aged care. During this time she has worked as a Registered Nurse in a number of aged care facilities, and has founded the Heather Hill Group, which provides integrated seniors’ solutions through:

Through Heather Hill Nursing Agency, Heather supplies specialists nursing staff to more than 150 aged care facilities, and hundreds to client’s in their homes through Heather Hill Home Care.
Brian is a Partner at CRH Law and has been a Practising Solicitor since 1983. Brian has a particular interest and expertise in law relating to older people, including the legal aspects of aged care and retirement villages, powers of attorney, advance health directives, wills, Centrelink rules and regulations, and family disputes and agreements.
Brian is Chair of the Queensland Law Society’s Elder Law Committee, an international Member of the National Academy of Elder Law Attorneys of America, and a part-time Member of the Queensland Law Reform Commission. He has a passion for Elder Law, and was one of the first lawyers in Queensland to recognise the need for specialists legal expertise to help older people and their families.
Jeremy is the Managing Director and Principal Adviser of Bravien Financial, a firm that is a member of the Financial Planning Association of Australia. Before starting his own financial planning business, Jeremy spent 11 years serving as an Officer in the Australian Army which culminated in active service in East Timor.
Since then, he has become a Certified Financial Adviser and accredited Aged Care Specialist. Together with his team, he has advised thousands of clients on financial issues including aged care, retirement and wealth transition.
Jeremy is passionate about providing technically excellent but easy to understand financial advice which ensures the best possible outcomes for clients. He is committed to the highest standards of integrity and transparency in managing clients’ affairs.
Cameron is a Partner and Co-Founder of Golden Years, a home transitions company that specialises in assisting seniors relocate from their family home into retirement villages, hostels or nursing homes. After over 15 years volunteering with Meals on Wheels Cameron had seen first hand the many logistical issues that families and their loved ones often experienced when relocating relatives from the family home into care environments.
Issues such as how to prepare the home for sale, how to select the right real estate agent, the stress involved in packing, moving and storing items, disconnecting utilities, redirecting mail and re-homing of pets, just to name a few. As a result of seeing these issues first hand and Cameron’s extensive background as both a licenced valuer and real estate agent he created Golden Years, a one stop shop for absolutely anything to do with senior relocations.
Claudia is a Registered Senior Psychologist and the founder of Transform Life in Change, with 15 years experience in helping people cope successfully with transition and change in their lives. She has counselled clients individually and also managed teams of psychologists, ensuring that they deliver quality counselling services.
Claudia is also a Registered Nurse, with in-depth experience and understanding of aged care. She has seen first-hand the emotional challenges that many families face in finding workable aged care solutions. Claudia’s role is to support families emotionally and to help them find practical solutions through counselling, case management, and a multidisciplinary approach.
Tom is a Lawyer who also holds university qualifications in mediation and conciliation. He is nationally accredited as both a Family Dispute Resolution Practitioner and a General Mediator. He has also been awarded the 2013 LEADR Practitioner Award for Excellence in the Practice of ADR.
Having mediated more than 2,000 disputes throughout Australia, including many relating to family law matters and family disputes, Tom has seen the damage that can be done to family relationships when conflicts escalate and remain unresolved.
Skilful mediation can allow families to discuss their concerns and reach an outcome that ensures good relationships are maintained. Tom believes that mediation does not replace other services, but should be offered in conjunction with sound health, legal and financial advice.
